NDIS Psychosocial Disability Support Melbourne

Psychosocial disability can affect a participant’s ability to manage daily activities, maintain social connections, access services, and work toward their NDIS goals. The functional impact varies between individuals and can change over time.

Hilda Care supports NDIS participants across Melbourne and Victoria living with the ongoing effects of a mental health condition. Our psychosocial support is practical, consistent, and structured around each participant’s individual goals and funded supports.

What the NDIS Recognises as a Psychosocial Disability

The NDIS does not fund a diagnosis. It funds the functional impact of a mental health condition, specifically the ways it limits what a participant can do day-to-day and over the long term.

A participant living with schizophrenia may have difficulty managing medication, maintaining a safe home environment, or attending medical appointments consistently. A participant with severe PTSD may experience significant distress in medical settings, on public transport, or in unfamiliar environments.

Conditions most commonly associated with NDIS psychosocial disability funding include schizophrenia, bipolar disorder, severe depression, PTSD, severe anxiety, and borderline personality disorder. What determines eligibility is the functional impact, not the diagnosis itself.

Who Hilda Care Supports

Participants Living with Schizophrenia or Schizoaffective Disorder

Support typically includes medication reminders, assistance in maintaining a clean and safe home environment, and help preparing meals when self-care routines have deteriorated. Following a hospitalisation or acute episode, support is focused on re-establishing daily structure in a gradual, consistent way.

Support workers assist with appointment preparation, travel, and follow-through on any recommended next steps.

Participants Living with Bipolar Disorder

During a depressive episode, support may involve assisting a participant with getting through daily living tasks, maintaining nutrition, and managing household responsibilities. During elevated periods, the focus shifts to maintaining consistent routines and providing a stable, structured presence.

Support workers adjust their approach based on the participant’s current phase to maintain consistent and appropriate service delivery.

Participants Living with PTSD and Severe Anxiety

Support workers assist participants with PTSD and severe anxiety in accessing medical appointments, using public transport, and engaging in community settings.

Daily routines are supported in a structured and consistent way. Where a participant has a complex trauma history, the same support worker is allocated across sessions to ensure continuity of support.

Participants Living with Severe Depression or Personality Disorders

Support workers assist participants with meal preparation, personal hygiene, and home maintenance where severe depression or a personality disorder affects capacity in these areas.

This support is funded under Core Supports. For participants with borderline personality disorder or other complex conditions, the same support worker is allocated over time to support a stable and effective working relationship.

NDIS Psychosocial Support Services

Mental Health Support

A mental health support worker provides practical, day-to-day assistance that supports recovery and daily functioning. Support workers are matched with participants on the basis of personality, communication style, and cultural background.

NDIS Psychosocial Recovery Coaching

Psychosocial recovery coaching is a dedicated NDIS support role, separate from support coordination and separate from clinical therapy. It focuses on building the routines, skills, and community connections that support long-term recovery.

This support is funded within the NDIS Capacity Building budget under Improved Daily Living. If it is not included in your current NDIS plan, it can be raised at your next plan review or through a change of circumstances review.

Community Participation Support

Social isolation is a common functional consequence of psychosocial disability. Re-engaging through community participation after an extended period of withdrawal involves structured, goal-directed community activities funded under Core Supports.

Support workers accompany participants to community groups, structured classes, and local activities. Consistent, supported engagement builds capacity over time.

Daily Living Assistance Under Core Supports

Psychosocial disability can significantly affect a participant’s ability to manage household responsibilities and maintain a functional living environment. Reduced capacity in areas such as meal preparation, grocery shopping, and general home maintenance is a documented functional consequence of many mental health conditions.

Hilda Care assists with household tasks and daily living under Core Supports funding. The goal is to provide support while building independent living skills, with the objective of reducing reliance on funded supports over time. For participants in shared accommodation, support extends across shared living arrangements.

Implementing NDIS Behaviour Support Plans

Some NDIS participants with a psychosocial disability also have behaviours of concern that require a formal behaviour support plan. Hilda Care implements behaviour support plans developed by NDIS-registered behaviour support practitioners, with support workers trained and supervised in line with the NDIS Practice Standards.

All support is delivered within our registration scope and consistent with the requirements of the NDIS Quality and Safeguards Commission.

How NDIS Funding Works for Psychosocial Disability

The NDIS funds psychosocial disability support across two primary budget areas.

Core Supports covers mental health support workers for personal care, daily living, community access, and transport. The NDIS Capacity Building budget specifically improved Daily Living funds for psychosocial recovery coaching, which builds skills and independence over time.

Whether you are agency-managed, plan-managed, or self-managed, Hilda Care works within your existing NDIS plan structure. If you are uncertain about what is included in your plan, a plan manager or support coordinator can assist you in reviewing your funded supports.

Frequently Asked Questions

My doctor says my condition isn't permanent. Does that affect my NDIS eligibility?

Not automatically. The NDIS considers whether the functional impact of the condition is likely to be long-term, not whether a clinician has used the word “permanent.” Supporting evidence that clearly documents how the condition affects day-to-day functioning is the determining factor.

What's the difference between psychosocial recovery coaching and support coordination?

Support coordination assists participants in setting up and managing their funded supports. Psychosocial recovery coaching focuses on the participant’s recovery journey, building routines, developing skills, and strengthening community connections over time. Both can be included in the same NDIS plan.

Does the NDIS fund therapy or mental health treatment?

No. The NDIS funds support related to the functional impact of a psychosocial disability, including support workers, recovery coaching, community access, and daily living assistance. Therapy, psychiatry, and medication are funded through Medicare and the broader health system.

Can I access support through Hilda Care if I'm plan-managed?

Yes. Plan managers handle invoicing on your behalf, and this does not affect how you access supports. Agency-managed, plan-managed, and self-managed participants can all access supports through Hilda Care.

What if my needs change between plan reviews?

A significant change such as a hospitalisation, a documented decline in function, or a major life event can trigger a change of circumstances review. Participants do not need to wait for a scheduled review date. A request can be submitted through the my NDIS portal or directly with the NDIA.

Can I receive psychosocial support and behaviour support at the same time?

Yes. Hilda Care can implement an existing behaviour support plan developed by a registered practitioner while also delivering day-to-day mental health support. Both are managed within our registration scope.
